The Collective Bargaining Agreement between CWA/IBEW and Verizon requires the Company to fund the programs enacted by the Work and Family Committees at $1.4 million per year, each August through 2030. 

Note: effective immediately, PLEASE EMAIL all Work and Family forms to:   verizon.benefits.team@verizon.com - 

• the due dates have NOT changed 

2026 Forms are now available for:

• Dependent Care Application and 2026 DCRF Reimbursement Form
  • Must be emailed by the 2nd Friday of the following month 
  • $100 per family per week
  • Must reapply with 2025 taxes prior to your May 2025 reimbursement request
• Pendant Program Application and 2026 Pendant Reimbursement Form
  • $60 a month paid quarterly (maximum 2 per member)

Health and Wellness and College Books have been suspended for 2026

Appeal Process:

Appeals must be received within 45 days of your written notification of denial of enrollment or within 45 days of a denial of reimbursement for expenses.

Appeals must be submitted to:      verizon.benefits.team@verizon.com 

• Please put:  Program Name - Appeal in the subject line of the email 
  • You must attach all necessary documentation when filing an appeal.
  • Include a valid reach number and current email address for a response.
