• Dependent Care Application and Reimbursement Form ~ Increased to $200 per week per family 

  • Must reapply before May reimbursement is processed

    • Must be postmarked by 2nd Friday of following month

• Pendant Program Application and Reimbursement Form up to 2 per member ~ $60 per month - paid quarterly
